Steps to complete your physical form
- Please print the physical form and fill out the entirety of the history portion (first page) before your physical appointment.
- Discuss the history portion with your physician during the exam.
- Completed physical form (2 pages) should be turned in to the Athletic Trainer prior to first practice.
- It is recommended that you complete your physical before you arrive on campus so there is no delay in clearance. If you are unable to complete physical before arriving on campus the Athletic Trainer can assist you with locations for physicals.
NDSCS uses online data entry system for your health information, this step must be completed by the below dates as part of the clearance process for participation.
Womens Soccer: July 22
Football and Volleyball: July 31
Baseball and Softball: August 15
Men’s and Women’s Basketball: September 15
Spring Semester Transfers: January 1
